ABSTRACT

What if orthodontics could truly serve growth and function? What if we were able to design, guide, and rebalance the entire maxillofacial pattern? Skeletal anchorage has profoundly transformed our approach to the
occlusal plane. Once largely excluded from orthodontic decision-making due to insufficient control of the lateral sectors; the occlusal plane has now been reinstated as a central criterion in treatment plan.
Regardless of the type of malocclusion, whether symmetrical or characterized by the full complexity of asymmetry, the occlusal plane must be carefully observed and analyzed as it often represents the functional
expression of an adaptive response to discrepancies between skeletal bases, and in certain cases, it requires reorientation. Mini-screws now enable three-dimensional modification of the occlusal plane’s orientation.
Through such control, it becomes possible now to redirect growth or reposition the mandible. Our current understanding of mini-screw biomechanics, including their potential “parasitic” effects, allows us — after more than 25 years of clinical application — to optimize their use. It is now feasible to integrate modification of smile position into the therapeutic project, while simultaneously striving to enhance occlusal function. The once seemingly unavoidable choice between stability, function, and aesthetics is gradually diminishing, and the dilemma between alveolar compensation and decompensation is no longer dictated only by our biomechanical
limitations. This presentation aims to propose new therapeutic pathways centered on precise control of the occlusal plane through skeletal anchorage with miniscrews.
